Before submitting your rebate:
1. Make sure you have the rebate coupon filled out properly. You can use the one at the link below, or the one supplied by the store where you purchased the Eliminator.
2. Make certain that the required items are included in your envelope (receipt, original UPC barcode)
3. Make sure the UPC code is actually the UPC code! It will start with 00381, and look like the example on the Rebate form.
